Hafren House Dental Practice is a well established dental surgery in the Derbyshire town of Alfreton.
The practice is over 40 years old and many of our patients have been coming to see us since they were children.
All the surgeries are equipped with state of the art dental equipment and our staff regularly attend continuing education courses to keep up to date and bring the latest techniques and advances in dental care to the practice.
When booking an appointment , we will try our best to offer you one as soon as possible at a convenient time. You can choose which dentist you would like to see at the practice , but it may not always be possible to arrange this because the particular dentist may not have any available appointments.
We are an extremely busy practice and appointments are very precious, to keep up our reputation of an efficient appointment system we ask if you are unable to keep you appointment we require 24 hours notice so that we can offer the appointment to another patient, possibly someone in pain.
If you miss or cancel at short notice 2 appointments within a 2 year time frame, we may not be able to offer you further NHS appointments.
At the end of your course of treatment, your dentist will discuss with you when you will need to see a dentist a again.
The practice follows guidelines issued by the National Institute for Health and Clinical Excellence (NICE) on how often patients needs to see their dentist according to their oral health needs.
As part of our efficient service we offer a Reminder system to inform patients and families when they are next due for a dental examination.
You will receive a TEXT message to your preferred contact number informing you when an appointment is due, then to contact the surgery at your convenience to keep up with regular examinations, however if you don’t want your reminder sent by text, then you will be given an reminder card following your last visit.
Under the Data Protection Act 1998 you are entitled to access your dental records. If you require a copy we can do this for you -all requests should be made to The Practice Manager in writing.
Strict confidentiality of patients records and information is maintained at all times. Patients records will not be passed onto any third parties without the patients express permission.
The practice provides an interpreting service to all NHS patients at no charge covering 26 languages and also BSL. Please speak to a receptionist for further information.
All staff are fully trained in CPR and in the use of an AED. Any accidents should be reported to the our trained first aider Lucy Fyfe, Emma Bates and the Practice Manager.
If you have any complaints about any aspect of our service or treatment you can speak or write to the Practice Manager. All written complaints will be acknowledged within 3 days. We hope that you will be entirely satisfied with your dental care and treatment and would be happy to recommend our services to others. If you feel we have do met your expectations in any way we would like to hear about it.
You can request a copy of the practice complaints procedure from The Practice Manager who will explain the procedure in detail and deal with any concerns you might have. We also have a suggestions box located at the patient waiting room, all suggestions will be kept confidential
All patients aged under 16 must be accompanied by a parent/guardian at every appointment and whilst in surgery.
We may ask you to fill in an up to date Medical History at each dental check up so we can keep all medication up to date, please make a note of their names and tell us about them when you attend.
Always keep us informed of any changes to personal details so we can make our service up to date and efficient.
